According to reports, Lindsay Lohan may be off the wagon yet again.

Over the weekend, the troubled young actress reportedly relapsed on alcohol during a trip to California for the Coachella music festival.

“She looked completely out of it. She couldn't stand up straight and she was frantically chain smoking cigarettes,” an eyewitness told In Touch Weekly on April 13. “In one of her hands she had a plastic cup with a clear liquid in it.”

On last week's episode of Lindsay, Lohan's OWN documentary series, Lohan came clean about a wine relapse she had suffered in 2013, following her stint in rehab earlier that same year. During her show, she explained to fans that she had just gotten into a relationship with someone who wasn't sober, and as a result, ended up drinking a glass of wine.

Immediately following the incident, Lohan kept things secret, but eventually decided to let her secret be known and move on.

"I just had so much guilt," she recalled. "It took me a bit to finally just be like, 'Why am I holding this in? Yeah, f**k it. I screwed up. I screwed up. And that's okay. But, now, what can I do so it doesn't happen again?' That's a part of the process. That's a part of life."

According to the source, Lohan's most recent reported relapse occurred during an April 12 performance by Kid Cudi, which she watched from a backstage area with her sister Ali and a couple of friends.

As they took in the show, one particular male in their group was making regular trips to the bar, and while Lohan stayed put, she reportedly made it no secret that she was drinking again. At one point, she was seen drinking a vodka soda with her friends.
